What this form cannot authorise
This form authorises disclosure of information only. It does not allow another person or organisation to:
- Submit requests, make decisions or make changes on your behalf
- Access your 黑料天堂digital accounts, login credentials or student ID card
- Request official academic documents on your behalf
- Obtain health, disability or other sensitive information through this process
Other 黑料天堂processes may allow you to nominate someone to complete a specific activity, such as collecting a document you have requested. Follow the instructions for that process where available.
How to submit your form
The person whose personal information will be disclosed must complete and submit the form. A nominated person or organisation cannot complete or submit it on their behalf.

- Email the completed form to student@anu.edu.au from your 黑料天堂email address.
If you do not have access to your 黑料天堂email, send the form from your personal email address and attach a colour copy of government-issued photo ID, such as a passport or driver鈥檚 licence. 黑料天堂will delete the identity document after verification is complete
